Updates to the CIB and CPM Applications Forms

Why we are changing the CIB and Application Forms

We recently updated our application process to enable online submission of all applications, including the Portfolio Evaluation Process (PEP) forms. These changes resulted in the need for significant revisions to the instructions/guidance in our CIB and Application Forms. As we reviewed these documents, we found a few other places where minor clarifications were needed to help ensure clear and transparent directions for all applicants and preceptors.

Please review the form instructions to ensure compliance with NARM policy

We recommend that you download these updated versions of the CIB and Application Forms. While NARM will still accept old applications (you do not need to start your application over again if you have already partially completed it), we recommend you review the instructions on the new forms to ensure you are in compliance with NARM policy.

We are here to help

NARM’s goal is to ensure equitable access to the application process. These changes were made with the goal of supporting all of you who are either in process or are a preceptor supporting a student. If you have any questions or concerns about these changes, we welcome your feedback. We commit to working together with all applicants and preceptors to ensure that no undue burden is placed on you during this transition to online applications.

Summary of updates

Below is a short summary of the updates to the CIB and Application Forms:

Application Process:

  • Applying through Certemy is now required, replacing mailed-in applications. (Note: for students who cannot access an online application, applications may be submitted on paper via mail. Because paper applications take longer to process, they may be subject to delayed processing.) The applications department is committed to review all online applications within 2 to 4 weeks of submission.
  • A new category for “suspension or expiration of applications” has been added.
  • A revised timetable for the application process has been established.
  • In the PEP Category, Phase one and two approval is recommended before Phase three.

PEP Forms & Documentation:

  • Personal reference letters are no longer required.
  • Form 200 Update: Charts are no longer needed; a new charting form is required.
  • Form 112a: COCs no longer need to be circled.
  • Form 112d: Updated to reflect birth time and exam time.
  • Form 112g: Details have been updated regarding signatures on this form and anywhere a preceptor’s signature is required.
  • Form 102: Clarification on co-primary for CNM/CM/UK pathways only.

Other Updates:

  • The criteria for sanctions against a CPM’s credential has been updated.
  • Definitions of forgery and falsification of documents have been updated.
