Hello,
I have a question about the OOC Birth Sites Form 230 page 2.
Should I write in the birth date for all clients that I list or should I only write in the birth date if I was the primary midwife at the birth? For example, if I have a client that I did 3 prenatals on, but that is all I did, would I still write in the date she delivered?
